Job Description: 
1. Conducts beneficiary registration & updates details in IT enabled application.
2. Screening of patients of age 30yrs and above for RBS and BP measurement for diagnosis of Diabetes mellitus, Hypertension
3. Checks & Records vitals in the prescribed format. Conducts laboratory tests (simple pointof care tests) as per the protocols and also as advised by the Medical Officer.
4. Initiates Telemedicine consultation for Newly diagnosed /Existing Cases of HTN/DM.
5. Gudiance to the patients on the nearest govt health facilities for availing further management.
6. Counsels and educates beneficiaries, as per specific need in personalizing, substance abuse, and physical activity.
7. Dispenses medication as per the advice of medical officer.
8. Develops rapport with Local Health Functionaries (Govt./PVT./NGOs)
9. Maintains detailed and accurate records and reports.
10. Takes appropriate action for Bio Medical waste management
11. Carries out any other relevant additional functions as tasked by the MO /Operations/CD team
12. Attend periodic trainings conducted by CD/Operations team.
